Introduction
This should include an overview of the person. For example: Mr Smith aged 89 years, was admitted to Robina hospital. He has a history of problems with psychotic depression and now states there is a man fishing in his stomach and he feels that he is unable to eat. Generally Mr Smith’s health is good and only visits the doctor for regular medical checkups. He is well-regarded and popular at the residential facility where he resides. His wife died almost 5 years ago and he has irregular contact from his sons, who both live interstate.
Mr Smith is a non-smoker, does not drink and the only medication he has is Rispiridone and Prothiadin at night. He has a history of psychotic depression. (Include symptoms here)

Assessment
This should include all bio-psycho-social (the whole person) assessment information. For Mr Smith this would also include a mental status examination and risk assessment. Include any other assessments you would think may be beneficial eg specific depression scales, or what Mr Smith may identify as being key issues for him. Provide an overview of the mental health status.
Mental Health Problem
If the person has a diagnosed mental health problem outline the problem but also include the persons’ strengths, capabilities and any external resources they have to help them with the problems in living they have. Provide a critical overview of the diagnosed disorder or possible disorder using references. Include treatment usually provided for the disorder and what the potential benefit and disadvantages this usual treatment may have.
Nursing Care
Include goal and nursing care

Objective- Mr Smith will eat small meals at regular times
Intervention- Obtain information on food Mr Smith enjoys
Provide small meals at times when the ward is not busy
Give Mr Smith plenty of time to eat the meal etc

Provide an overview of the outcome of the care given and sum up the case study

He had a history of psychotic depression and became moderately depressed after the surgery. Mr Bob Smith is brought to the mental health clinic by his case manager. Mr Smith lives in a residential aged care facility “Placid Palms”. Mr Smith has a number of concerns: he thinks that there is a man fishing in his stomach and so feels unable to eat and complains of constipation; he also believes that his room at the aged care facility is constantly being searched, and that the cleaners there are in charge of medications and dispense him the wrong tablets. Recently the cook gave him a lavender spray for his room but he believes this is a poison spray. He also believes that a mate of his is being abused in the room next to his.
The case manager tells you further that Mr Smith is well thought-of, and popular at the facility, where he is polite, reads the newspaper, and is up-to-date with current affairs. Mr Smith was born in Argentina in 1918, but his parents moved to Poland where he later became a prisoner-of-war. When he was released he moved to Australia, settled in Lismore and married his wife, Phoebe, who died 5 years ago. They had 2 sons who both live in Adelaide. Neither of his sons is close to Mr Smith. They accuse him of being violent to them and their mother while they were growing up, and have only irregular contact with him.
Two years after his wife died Mr Smith was diagnosed with psychotic depression, and this led to two admissions to the psychiatric unit where he was prescribed a small dose of Rispiridone, and Prothiadin at night.
