Instructions: Using the format below (or for your first homework the Beautiful Mind report template; cf. Kate’s Secret or Good Will Hunting example reports) as a guide complete the five DSM-IV-TR Axes. And then in section 4 list the DSM-IV-TR diagnostic criteria found in the Quick Reference (QR) for the primary/principal diagnosis for the assigned video character, and then address each criteria separately. You must address ALL of the Criteria for the primary diagnosis (using “N/A” for minor criteria being acceptable, but not for major/capital letter criteria). You must use and follow the DSM outline form in the QR. If you want the quality points you should also list secondary diagnoses in the Axes, but you do not have to list or address their criteria. Then using either essay or outline form in you report’s section 5 explain where the primary disorder came from (= Etiology or root causes) according to both the movie AND the textbook (or another reference), and then in section 6 explain the psychological treatment the character received or should have received according to the textbook or other references. Approximately half your report should be Diagnostic Documentation in sections 3 and 4, a fourth on Etiology in section 5 and a fourth on Treatment in section 6 (cf. Kate’s Secret report). These six sections of most reports earning “A” grades (excluding Personal Application) have at least 5 double spaced pages (though I prefer line spacing to be set at 1.5 lines); but note that reports over 10 pages will be considered too long and will cost you points. Also, note that failure to use the official DSM-IV-TR outline form for diagnostic criteria found in the QR will result in the report being returned to you ungraded. Any revised reports subsequently turned in will still lose late points, but if left unrevised will result in a grade of zero
