Case Study: 75-year old gentleman who has had a stroke
Mr Roberts experienced a cerebrovascular accident four weeks ago. It happened during the night, and so it was not known about until his wife found she was unable to rouse him in the morning, and called an ambulance. He was not suitable for acute thrombolytic therapy as the time elapsed since the CVA was not ascertainable.
Prior to admission, Mr Roberts was mildly hypertensive, and taking statins. Somewhat overweight at 5’10” and 90kgs, he used to smoke about 20 a day but had given up ten years ago. He enjoyed going to his local pub for ‘early doors’ three times a week, where he would meet up with long-standing friends, and drink two or three pints. Prior to retiring at the age of 65, he was the manager of a mans’ clothing store in the town centre. His leisure activities included looking after his garden, coarse fishing in the local river, and using the internet.
On admission, Mr R was unconscious, and found to be in atrial fibrillation (AF). He regained consciousness over the next 24-48 hours, and therapy was started to control the AF. Once stabilised, he was transferred from the Medical Assessment Unit to the Stroke Unit. He was also started on warfarin.
Mr R was found to have sustained ischaemic damage to the left hemisphere due to occlusion of the left middle cerebral artery (MCA). This resulted in persisting impairments of right hemiplegia, diminished sensation to the right side, and mild expressive aphasia. Initially he had some dysarthria and swallowing difficulties, but these largely resolved. He was incontinent in the early acute stage of recovery, but now has control of bladder and bowels.
