Choose an issue that interests you (such as incivility in healthcare settings, ineffective communication between doctors and patients, hospital-acquired infections, medical errors, teen pregnancy, obesity, depression, crime). After reviewing prior evaluations, propose a new program to address the issue in Flint, Michigan or another local area. Develop an evaluation plan to answer the implementation and effectiveness questions.

The first 6 pages of the assignment should be typed, double-spaced, with Arial point font, and with margins no larger than 1 inch on the left, right, top and bottom.
The program materials should consist of the following:
o Logic model, including implementation and outcome objectives (1 page)
o Process evaluation tools, i.e. surveys for quantitative evaluations; interview/focus group guides for qualitative evaluations. (1 page; Pages will vary depending on the length of the tools.)
o Outcome evaluation tools, i.e. surveys for quantitative evaluations; interview/focus group guides for qualitative evaluations. (1 page; Pages will vary depending on the length of the tools.)
o Budget, cost per subject. (1 page; Pages will vary depending on the length of the budget form.)
