Assessment Case Study – Anna Grande is a 30 year old G4 P3 Somali woman, newly arrived to Australia, presented for her first antenatal clinic visit at 15 weeks gestation. anna was fit and well and had no significant past medical, surgical or obstetric history. Her previous pregnancies had all been without complication and progressed to normal vaginal deliveries.
Anna’s blood pressure at first visit was 140/90 mmHg. Subsequent monitoring of her BP revealed persistent hypertension and at 29 weeks gestation she was admitted for stabilisation with drug therapy. Within the first week of her hospitalization Anna was commenced on ‘second line’ drug therapy.
At two weeks of hospitalisation the drug therapy remained in place and Anna had developed significant proteinuria. During this period the fetal monitoring revealed intrauterine growth restriction (IUGR). By 31 weeks gestation Doppler flow studies revealed absent end-diastolic flow. At this point Anna was delivered immediately by Caesarean section.

A. Describe your findings of Anna’s condition and explain how you would differentiate hypertension in pregnancy from other hypertensive conditions. D. Develop an education plan for Anna during her antenatal stay in hospital in relation to her and her baby. YOu need to include the information that would be specific to her condition.
