Young lady Named Miss XYZ come in for Xray in ABC Imaging Services. She is overweight,I tried to put lead apron(only one lead apron with Velcro missing) by making knot for her PA(posterior anterior) Chest Xray but for lateral I couldn’t fit the lead apron as she is big and if Velcro was not missing I could use so I proceed to do chest xray without putting lead apron.
History: Patient was presented with cough since 2 weeks with no fever and yellow sputum. ( feel free to change patient history (if that makes easy for you to write) but not too dramatic. Mind You,She is young.

Principles of Beneficence: requires that we take positive steps to benefit others: to contribute to their well-being ,if we fail to uphold our commitment to act beneficently. Chest Xray was necessary even it will harm (more dose without lead protection) in the provision of health care ( chest xray thus radiation dose) is justified if and only if one is acting, firstly, to benefit the patient (eg restoring health)
Principles of Justice: Practices is without adequate and out of date lead protection. There is negligence and total discrimination against overweight people.
Principle of non-maleficence: Primum non nocere: ‘above all: do no harm’ (from the Hippocratic Oath)
i.e. we ought not to intentionally inflict harm on a patient (eg by stealing from them, insulting or humiliating them, or neglecting them)
We were making young girl embarrassed it was not her fault being a overweight it is duty of care of the practices that they should provide lead protection for each and every individual.
Both principles (beneficence & non-maleficence) require us to avoid harm (unless it is unavoidable when acting to benefit the pt) BUT HAVING A bigger lead apron or apron with Velcro could avoid extra dose to the young girl however in that situation I have to proceed with x-ray as I had no option and I made aware that this is tiny dosage and she was happy to do xray without the lead apron.
Handling of Ethical Dilemma: In real scenario: I apologizes as I couldn’t wrap lead apron around her waist she was happy to do xray without lead apron. BUT please I reckon this is not enough….
