Students are required to select a real life case study from their own clinical experience, and to analyse this case. The case is to involve a patient who you have provided care from in the area of practice you have utilised throughout the weekly themes. The case is to be de-identified so no individual can be identified.
Students are to use scientific and qualitative evidence to explore the developments in nursing practice relevant to their case study.
The case study is to include (this is not in a priority order):
?X The medical conditions of the patient and the underlying patho-physiology.
?X A comprehensive nursing assessment of this patient, this should include an evidence based discussion on the nursing assessments utilised.
?X A discussion on the tools utilised to detect any deterioration in the condition of the patient. Strategies that were utilised to identify and manage any change or deterioration in the condition of the patient.
?X Consideration of evidenced based referrals on discharge for this patient, this should include any appropriate admission avoidance and chronic care programs.
The case study report may contain headings. There should be an introduction, body and conclusion to the report as a minimum.
