I am to write the first part of the as a dental student evaluating the different practice models (faculty member of dental school, solo general dental practice, practice as an associate, an employee of a dental care group: public health service or private dental corporation). my goal is to demonstrate to me that you understand each of the models/types of practices (public health dentistry, employee in corporate style dentistry, establish own practice, residency, armed services, etc. – there is a list of each of the models). Each is to be described based on the following: 1)ease of transition to each model from dental school, 2) ability of each model to reach oral health care needs of patients 3) your responsibility (legal, financial, business) as a dentist. 4) your ability to continue to develop your skills, 5) your ability to achieve your financial goals 6) your ability to achieve your personal goals.

As a new graduate from dental school, it is important to know that there are many types of career path to choose from. However before I choose my career path, it is important for me to determine what are my interests, goals, financial obligations, life style,amount of experience I have and so on, in order to choose my career path. There are different models of health care available to us to choose from. These include but are not restricted to the following: faculty of dental school, Solo general dental practice, Practice as an associate, and an employee of a dental care group, which could be public health service or a private dental practice. All these paths have benefits and set backs. So it is very important to research each model to determine if it suits me the best.
The first model to consider is to practice as a faculty member of dental school. it is more difficult to transition to this model as an inexperienced dentist. Because you are expected to have experience and knowledge to teach dental students. Therefore it is a better transition to this model only after you have been more experienced
As a faculty of dental school you are expected to provide new findings for future dentistry by doing research, in order to meet oral health care needs of the patient. However unfortunately the option of directly providing dental care to patients is significantly reduced.
your responsibility is to provide research articles which are published in academic journals, however this is also dependent on dental schools some may be more focused on research than others. you are responsible to mentor students in order to develop future dentistry.
You will continue to develop skills since you are around many others that have higher knowledge and experience that you can learn from.
As a faculty you will often obtain benefits including paid vacation, health insurance, life insurance, malpractice insurance, continuing education, and so on. I will earn competitive salary. Yet you may earn lower salaries than associate or solo practitioner due to your reduced ability to practice. Overall you have a less of a financial risk since you are not obligated to support a practice and pay back loan. You also have the advantage of immediate income to support your financial goals.
Depending what your personal goals may be. You have the option of enjoying the best of both worlds. Meaning you have the option of teaching, research and mentoring future dental students. Also you can continue to see patients in clinical setting. Therefore depending on your goals you have both options available to you.
As a solo general dental practitioner. It is difficult to transition from dental school directly to solo practice. Since you need to have the money to support a practice. As a recently graduated dental student many of us have burden of paying back our student loans so it would be even more stressful to borrow additional loans to buy a practice. In addition we need to have practice management skills, to support a practice. We need to have the speed and experience with seeing increased number of patients. These all come with more years of experience. Therefore, it is more difficult to transition to solo practitioner model directly from dental school
You have increased ability to meet oral health care needs of patient by focusing on each patients to determine problem and preventive measures individually for each patient and reinforcing it to them with help of your hygienist and assistants. Therefore since you have higher ability to control oral health care of patients, you are more responsible toward each one of your patients.
You have a higher financial, business and legal responsibility as a solo practitioner. You have the greatest financial risk. You are managing a practice that is solely on you and you have to have the skills to manage it financially. You should be skilled not only in dentistry but also in business and practice management. You are responsible to manage your staff, market your practice, monitor cash flow, coming up with fee schedule, negotiate contracts, manage your overhead expense and so on. If any problems with collections you are the only one responsible for it.
As a solo practitioner you will continue to develop your skills in dentistry by taking continuing education courses, and by interacting with your colleague specialists. also increased experience by seeing more and more patients. Your financial skills will also continue to develop over years as you get more experienced in managing a practice and your employees.
You have a benefit of having higher ability to achieve your financial goals due to having greater earning potential and profitability as a solo practitioner. You have significant asset (patient base) which is your practice, at retirement. Number of patients that returns for continuing care is really important at that point. You also have greater financial responsibility to plan for your retirement.
You have a high ability to achieve your personal goals because you have the highest level of control over all features of your practice. This includes your ability to focus on preferred clinical procedures. This will help you reach your personal goals as a practitioner in a long run.
Practice as an associateship:
This is a choice that offer a chance of easier transition directly from dental school. This is because associateship gives the oppourtunity to work on clinical skills and spend time with an experienced dentist in order to develop better skills in seeing patients on your own. At the same time of developing these skills by working with an experienced dentist you have benefit of making money. However there are two different types of associateship:
With out practice equity: no matter how long you work there and what you do for that business. You are not going to have practice equity another words you are not going to own part of that practice, you are going to be an employee directed by your employer.
With practice equity: as you associate in that practice you are developing equity in that practice. You are buying into that practice over a certain period of time, by investing and becoming a partner in the practice.
In an associateship with equity you have higher ability to meet oral health care need of patient than the one with out equity. The reason for that is for an associateship with equity since you are buying into part of that practice, you have higher responsibility and ability to effect changes in dental office regarding the patient?s oral health needs. However in both with and with out equity, since it is a professional team approach and consultation with shared responsibility for patient needs. There may be conflicting opinions regarding different ways of approaching patient?s oral health care needs.
Your financial, legal and business responsibility is much higher in associateship with equity than one with out equity.When you finally get to a point that you have practice equity, they will cut out that monthly payment. They will pay you a certain percentage of your collections, so now you are being paid based on what you produce. Not on just the fact that you are there. If you are getting paid on what you produce you need adequate number of patients, there needs to be enough number of patients.. in addition you require initial funds to buy into that practice. Therefore you have higher financial responsibility than the one with no equity. In a practice with out equity you have no financial risk or initial capital requirement, which also means you have reduced earning potential.
You have high ability to continue to develop your patient skills in both with and without equity since you have oppourtunity to work with experienced dentist and learn from their knowledge and experience. Yet you have higher ability to develop your practice management skills in an associateship with equity since you have higher responsibilities to manage business part of your practice due to investing into that practice, this will help you develop your practice management skills over time.
Ability to achieve your financial goals: In the beginning of associateship, both models of with and without equity you are being treated as an employee and have immediate income for being there. However if you decide to buy into the practice ( associateship with equity). You are being paid a certain percentage of what you collect. This brings better oppourtunity for you to reach your financial goals, however there are risks to it as well because you need to make sure you have adequate number of patients.
Ability to achieve your personal goals: you have higher ability to achieve your personal goals in an associateship with equity. This is because you are investing into the practice and buying part of it which gives you more option and freedom to develop your own personal goals as a practitioner in that practice. In an associateship without equity you are usually provided with continuing education courses and vacation time. However, you have limited ability to make changes in the dental office because you are treated as an employee. Therefore this gives you less freedom to achieve your personal goals in that practice since you have less voice.

Public health service:
The ease of transition directly from dental school: this provides easier transition directly from dental school since you have an oppourtunity to practice with experienced dentists and dental team members in a group practice setting.
This model provides great oppourtunity to meet oral health care needs of patients by providing oral health care to underserved population, military personel and so on.
My responsibility as a dentist includes both clinical and non clinical activity, including increased management responsibility, and non clinical obligations related to federal career.
I will have excellent oppourtunity to continue to develop my skills by utilizing the exceptional continuing education oppourtunites offered by military branches to complete general practice residency, or specialty training as part of their military service.
I will have better chance of achieving my financial goals since I will be offered Salaried positions and excellent benefits, with retirement plans. Loan repayment may also be available.
Utilizing this model it would be difficult to achieive my personal goals since I would have to move throughout the country, to serve the areas of needs. This would make it difficult for me to manage my personal and family goals.
Private dental corporation:
Transition from dental school to private dental corporation is relatively easy since I will have the opportunity to work with other dental professionals and learn from their experience.
Ability to meet the oral health care needs of patients may be limited for me in some cases. Since I have to follow their guide lines and in some cases I may not agree that their guide line is best treatment for my patient.
I will have less of a financial risk and responsibility as a dentist since I will be an employee at the corporation and I will be paid a certain amount of salary. I do not have to worry about managing the corporation. So overall I have less of a responsibility while working there.
I will have the opportunity to develop my skills specially early on right after dental school graduation. However overtime after becoming skilled practitioner, since there is guide lines as to what type of procedure we can and cannot do, will limit me from improving my skills over a long run.
My ability to achieve my financial goals will be limited since no matter how much my production is for the company, I will be paid the same amount of salary.
My ability to achieve my personal goals may also be limited since I do not have the freedom of having my own office and deciding my own time off with my family.
As a dental student who has just graduated from dental school, I plan to enter perio residency program after dental school graduation, because I find periodontology the area of dentistry that interests me the most. Following my graduation from perio residency program I decide to become an associate with practice equity. This model allows me to work with experienced dental professionals and learn how to manage a practice and treat patient in a practice setting, therefore it helps me transition much better. In addition it gives me an option to buy into the practice and invest while working. This will also reinforce me to increase my speed and efficiency in order to increase my production to increase the percentage of my collection. This will also challenges me to market my practice in order to increase my number of patients to increase my production. Overall I think this is a better option for me as a new perio graduate from dental school, because I will have oppourtunity to work with experienced dentists, practice management skills and earn money to manage my financial obligations such as loan repayment.
